Enlarged endolymphatic duct and sac syndrome (EVAS) is the most common inner ear malformation, detected by radiologic examination associated with recurrent, progressive sensorineural hearing loss during childhood. Nevertheless many cases have been reported but the unilateral case is rare. Recently we experienced the case of unilateral EVAS associated with auditory neuropathy. The midpoint diameter of left endolymphatic duct was 2.2 mm, and the dimension of left endolymphatic sac was 14¡¿4 mm in MRI scan. In audiologic test, left ear showed the normal otoacoustic emmision response and abnormal response in auditory brain response. This case lead us to reelvaute the worthwhile diagnostic and observational protocols for EVAS with hearing loss.
